Research and Creative Activity:

My program of research focuses on improving nursing practice, patient safety and clinical outcomes. My first study explored methods and processes for expediting the transfer of new knowledge into nursing practice (Donaldson & Rutledge, JONA, 1993). With the emergence of nursing's quality initiative and the nation's safety strategic priorities, my work has focused on advancing nursing's capacity for quality measurement, benchmarking and clinical quality/safety research through the development of a statewide nursing quality data repository (Donaldson et al., 2001). The California Nursing Outcomes Coalition (CalNOC) was launched in 1996 through a partnership between American Nurses Association\California and the Association of California Nurses Leaders (ACNL) and the CalNOC Steering Committee was formed. CalNOC's subsequent proposal to the American Nurses Association, in response to an American Nurses Association Request for Applications, was funded and in 1996 CalNOC became one of American Nurses Association's six original state-based quality indicator research and development projects (Donaldson, Brown, Aydin & Burnes Bolton, 2001; Grobe, 1998; Redmond, Riggleman, Sorrell & Zerull, 1999; Sheehy, et. Al, 2000). The CalNOC Project has successfully completed a feasibility study (1995-1996), a pilot study (1997-1998) and is in the midst of an indicator expansion and research program development (1999-2009). CalNOC has successfully confronted inherent challenges to the integrity of multisite quality report card efforts identified by Jennings et al (2001) including indicator measurement standardization, feasible data retrieval, strategic relevance, and linking the work to the contemporary demands of clinical nursing. CalNOC's mission is to, 1) build and sustain the CalNOC statewide nursing staffing and quality database repository; 2) conduct research to advance evidence-based administrative and clinical decision-making; and 3) provide data to resolve public policy and clinical dilemmas in patient care delivery influenced by nurse staffing and quality. Since its inception, I have served as Co- Principal Investigator for CalNOC, the largest ongoing statewide prospective nursing quality measurement research and development project in the nation and a major contributor of data to the American Nurses Association's National Database for Nursing Quality Indicators (NDNQI). As the CalNOC database has grown into a robust source of nurse staffing and patient outcomes data, I have advanced proposals and initiated highly collaborative studies leveraging the CalNOC infrastructure to better understand the associations between nurse staffing and patient care quality/safety (AHRQ R01 Unit Based Nurse Workload Impacts on patient Safety), and explored using CalNOC to expedite the translation of evidence-based practice improvement in participating hospitals (AHRQ CalNOC Partners to Reduce Patient Falls Study). In 2004 the Center was funded by the Gordon and Betty Moore Foundation to develop and lead a project using CalNOC capacity for nursing quality measurement to evaluate the impact of the GBMF $110 million multifaceted intervention to improve nurse related patient care quality and safety in 42 Bay Area hospitals. The UCSF CalNOC GBMF Nursing Initiative Project ($1.4M—Year 1) was launched in April 2004 (Donaldson as PI) and expanded CalNOC's research, administrative and data management enterprise. In 2005, The Robert Wood Johnson Foundation invited the me to submit a proposal to evaluate the impact of their national Rapid Response Team Learning Network grants and I recruited Drs. Mary Scott RN, PhD., Susan Shapiro RN PHD, and Joanne Spetz PhD., to participate in this exciting project that integrates quantitative data and onsite interviews in 18 hospitals drawn from 13 states, to explore the impact of RRT implementation through the eyes of the nurse. This exciting project is now in progress.

My programs of research and instructional design innovation are well aligned with patient safety priorities at the NIH and the US PHS. In response to emerging RFAs, I developed and submitted three new projects in 2001 and two of those projects, an R01 building on the CalNOC project and an interdisciplinary education project, were funded. Currently in progress, those projects are described in detail below.

Collaborative Alliance for Nursing Outcomes Project (CALNOC)
.

A collaborative project between the American Nurses Association\California and a Association of California Nurse Leaders, supported by myriad donations, including grants from the American Nurses Association, Kaiser Permanente, and California Healthcare Foundation. I served as PI/PD for three successful proposals to the American Nurses Association\California Coalition for Nursing Outcomes 1996, 1997 and 1999.

The aim of the Collaborative Alliance for Nursing Outcomes (CALNOC) Project is advance patient safety and quality of care. In 1995, CALNOC indicators were selected from among the first American Nurses Association (ANA) Acute Care Indicators to be advanced for testing. Additional indicators have emerged based on recommendations from CALNOC sites and in response to regulatory imperatives (e.g, restraint prevalence). Today, over 90 acute care hospitals are currently collecting CALNOC data statewide. The CALNOC data repository currently contains 13 consecutive quarters of staffing data from 400 medical, surgical, step down, and critical care units. The following nursing quality indicators are the focus of CALNOC's current data collection efforts:

  • Patient days, direct care hours, skill mix, and RN education level and certification
  • Patient falls-risk, incidence, and consequences
  • Pressure ulcers-risk and prevalence; stage and nosocomial
  • Restraint Prevalence-type and clinical justification
  • Patient satisfaction with pain management, patient education, and overall nursing care.

CALNOC uses a rolling site accrual process and a voluntary, convenience sampling method in an effort to enroll hospitals with a strong strategic commitment to support the requisite data collection and repository growth over a sustained period. CALNOC data are hospital generated and collected at the patient level or patient care unit level. All CALNOC sites use the CALNOC Acute Care Codebook to ensure data collection precision. CALNOC sites receive quarterly aggregated descriptive reports, which currently fuel their internal and external benchmarking. Future reports will be enhanced with relational data, expanding the utility of current descriptive findings.

2008- NOW (Principal Investigator)
“Impact of Medical Surgical Acute Care Microsystem Nurse Characteristics and Practices on Patient Outcomes”
Interdisciplinary Nursing Quality Research Initiative, a national program of the Robert Wood Johnson Foundation. $299,971 The aim of the California Nursing Outcomes Coalition (CALNOC) INQRI project is to develop an empirically derived predictive model examining individual and collective effects of unit level nurse workload, staff nurse characteristics and selected risk assessment and preventive intervention processes of care on variance in nurse sensitive outcomes of acute care medical-surgical units. The study uses “community-based action” research methods in a non experimental design that integrates quantitative and qualitative (observational) methods. This study will be conducted in 100 medical surgical acute care units drawn from hospitals of the CALNOC network over a 24 month period. The outcomes studied include, (1) incidence of falls and fall related injuries, (2) prevalence of hospital acquired pressure ulcers (HAPU), (3) prevalence of medication administration errors, and (4) prevalence of peripherally inserted central catheter (PICC) associated blood stream infections. As a secondary aim, we will add a new measure, Hospital Acquired Stage 3-4 Pressure Ulcer Incidence, to the CALNOC dataset. Integrating this publicly reported indicator into the CALNOC dataset will enable timely study of the association between HAPU prevalence and incidence, and exploratory investigation of the sensitivity of both HAPU measures to variation in nurse workload, staff expertise and selected clinical risk assessment and prevention practices, and the predictive power of early stage prevalence for late stage incidence. The aims of the CALNOC INQRI study are responsive to the goals of RWJF’s INQRI, advancing understanding of how nurses contribute to and improve quality of care. Project dissemination will leverage CALNOC’s regional, national and international partners and policy makers, and will reach out to diverse stakeholders through briefings, forums, presentations and publications.

2006-Now (Principal Investigator)
“Lucile Salter Packard Children’s Hospital, Impact of EMR Nursing Documentation on Medication Administration Accuracy Study.”
Funded by Pediatric Research Program $35,000. This study will establish the baseline for medication administration accuracy using current systems and then repeat the measure of medication administration accuracy following LINKS Phase II implementation of the nursing documentation system. Principal aims of this study will be to: • Describe the baseline prevalence of errors in medication administration, both type of error and number of actual errors, in relation to the number of dose opportunities for error on representative LPCH critical care and medical surgical units. • Describe the prevalence of errors in medication administration, both type of error and number of actual errors, in relation to the number of dose opportunities for error on acute care medical surgical units post-LINKS implementation. • Compare medication administration errors identified through the CalNOC medication administration accuracy indicator observation method with the incidence of errors captured by the LPCH unusual occurrence reporting system, noting difference in both when compared to reported benchmarks in adult medication safety literature.

“CalNOC Partners to Reduce Patient Falls Project” AHRQ #1U18HS1370401 ($1.2 million/ACNL Prime)

The primary aim of the AHRQ Funded (Grant # U18 HS13704) CalNOC Partners for Quality TRIP To Reduce Patient Falls Project, a four-year quality improvement demonstration project, is to use evidence from the reported literature and the California Nursing Outcomes Coalition statewide repository to reduce the incidence of patient falls and severity of fall-related injury in California Hospitals. The proposed project builds on the infrastructure of the California Nursing Outcomes Coalition (CalNOC). CalNOC engages California acute care hospitals in voluntarily reporting standardized nurse staffing, patient falls, and fall-related injuries, as well as other quality indicators, in a collaborative repository development and benchmarking project using American Nurses Association's quality indicators. CalNOC is the largest of the American Nurse's Association (ANA) nursing quality measurement research and development project and a major contributor of data to the ANA's National Database for Nursing Quality Indicators (NDNQI). This demonstration project expands and advances CalNOC's efforts to use its quality benchmarking infrastructure as a vibrant network to expedite the transfer of evidence-based knowledge into practice as the basis for improving patient care quality and safety.

Reducing patient falls in acute care hospitals in California may be viewed as a first step in reducing patient falls nationally. Analysis of 13 quarters of CalNOC prospective hospital-generated patient falls risk assessment, incidence, and injury data reveals wide variation in fall rates, ranging from less than 1.0 to 13.0+ per 1,000 patient days and nearly 50 percent of patients with unknown or undocumented risk assessment status. A majority of CalNOC sites (76 percent) report using "home grown" fall risk assessment tools with unknown predictive validity. We believe these findings may be representative of processes of care nationwide and present a clear professional mandate to reduce patient falls in acute care hospitals by translating evidence-based knowledge of falls risk assessment and the efficacy of fall prevention interventions into nursing practice.

We have recruited 92 medical surgical units drawn from 33 hospitals and are engaged in coaching their individualized falls improvement projects. We are concurrently working with ANA and the NDNQI to launch this project in a web-based version in 2005.

2005-2008 Donaldson, N. (Principal Investigator) A solicited Grant. Robert Wood Johnson Foundation. National Rapid Response Team Initiative Evaluation Study, $224,023 . This solicited proposal engaged Donaldson and her team of co-investigators (Drs. Mary Scott, RN, PhD, Susan Shapiro, RN, PhD., Joanne Spetz, PhD.) in working with RWJF grantees to capture quantitative measures and qualitative data from their hospitals and can be leveraged to understand the impact of RRT implementation both clinically and from the perspective of the registered nurse. Donaldson's team has collected interview data from site visits to 18 hospitals in 13 states. Data analysis is now in progress and findings are expected to be released in early 2008.

A secondary aim of the study will be to develop and pilot test courseware for training nurses to be naïve observers as required for data capture. CALNOC has developed preliminary training materials; however, to ensure a high degree of inter-observer reliability, a systematic and formalized training program will be developed and upon completion of this study, deployed statewide as the CALNOC training standard.

2004-2006 Donaldson, N. (PI and Co- Investigator) Moore Foundation Nursing Initiative—CalNOC Baseline Outcomes Measurement Project ($1.4 M – Year 1; 750K – Year 2

This Phase I project focuses on 7 key objectives that are expected to cumulatively expand CalNOC's measurement, administrative, and data management capacity, strengthen its strategic partnerships and engage target hospitals in CalNOC enrollment and readiness for ongoing expanded data capture. The Target Outputs/Deliverables include:

#1 Target Output/Deliverable--Expand and strengthen CalNOC strategic partnerships and its administrative, research and data management infrastructure to establish the expanded baseline and ongoing outcome measurement of the impact of the GBMF Nursing Initiative in acute care hospitals in 5 Bay Area counties.

#2 Target Output/Deliverable—Integrate the CalNOC Steering Committee and Data Advisory groups, and expand stakeholder representation, to form the CalNOC Advisory Group, with the explicit aim of engaging authoritative national and regional interdisciplinary experts and clinicians in advising the CalNOC GBMF Project Team and the CalNOC Executive Team, to expedite and advance the strategic aims of the project

#3 Target Output/Deliverable—Recruit non-CalNOC hospitals in the target Bay Area counties to voluntarily join CalNOC and retain 95%+ or N=42 of the target acute care hospitals in the CalNOC database for the duration of the GBMF Project.

#4 Target Output/Deliverable—Expand CalNOC capacity for unit-based primary data collection, data management/analysis; transmission to the American Nurses Association (ANA) National Database for Nursing Quality Indicators (NDNQI); and effective reporting of data to hospitals to optimize the impact CalNOC analyses and benchmarking improvements in nursing-related patient outcomes.

#5 Target Output/Deliverable--Aggregate 24 quarters of current CalNOC data for hospitals within the 5 Bay Area Counties to provide a preliminary BASELINE for existing CalNOC indicators and assess focal opportunities to improve unit level nursing-related patient outcomes related to staffing effectiveness, risk assessment and prevention of patient falls, risk assessment and prevention of hospital acquired pressure ulcers, restraint use prevalence, care along continuum from in hospital-based acute to post acute care, and in collaboration with the NDNQI, identify opportunities to improve RN work satisfaction.

#6 Target Output/Deliverable-- Aggregate 24 quarters of CalNOC data for hospitals within the 5 Bay Area Counties to provide a preliminary BASLINE for patients 65 years+ for existing CalNOC indicators and explore differential impacts of nursing staffing on patient outcomes when sample is stratified by age.

#7 Target Output/Deliverable—Expand unit-level prospective outcome indicator measurement capacity for target Bay Area hospitals, building on NDNQI, NQF and IOM recommendations, design and conduct pilot data capture feasibility studies, develop design to capture evidence of Nursing Initiative impacts on knowledge, policies/procedure and practices; and integrate OSHPD outcomes data for selected “hospital/service level” outcomes.

In addition to achieving the target objectives 1-7 for Phase I, it is expected that the Moore CalNOC Project Team will develop and advance a Phase II proposal, during Phase I, to build on the ongoing measurement of nursing-related patient outcomes with rapid cycle interventions to expedite the translation and adoption of evidence-based Best Practices to improve patient safety and outcomes.

ORIGINAL ARTICLES IN ONLINE JOURNALS

In addition to publications in a wide range of well-established peer reviewed journals, in my role as Founding Co-Editor of the Online Journal of Clinical Innovations, I have been first, second, or contributing author for every article published. The Online Journal of Clinical Innovations is a peer reviewed scholarly publication (Murphy, S.C, 2001, Nursing research journals: An annotated guide, Serials Review, 27[1]. 26-44; Pravikoff & Donaldson, 2001a) that engages subject matter experts, as well as its Editorial Board, as collaborators and reviewers in developing, critiquing, revising and finalizing manuscripts that synthesize knowledge for practice using a comprehensive and innovative integrative review method. At the direction of the Editorial Board, Donaldson & Rutledge have served as principal authors during the first years of Online Journal of Clinical Innovations publication in order to develop, refine and reliably replicate the knowledge synthesis approach and integrative literature strategies that comprise the journal. Beginning in 2001, having accomplished the developmental aims above, with the support of the Editorial Board, Donaldson & Rutledge began mentoring and facilitating the work of other authors (Bowden & Greenberg, 2001).
