What is Valencia Health Services (VHS)?
VHS is a pediatric and young adult primary care clinic. We see children
from birth to age 21 years.
When are you open?
Valencia Health Services is open:
Monday -- 8:30 a.m. - 12:30 p.m. and 1:30 p.m. - 5:30 p.m.
Tuesday -- 8:30 a.m. - 12:30 p.m.
and 1:30 p.m. - 5:30 p.m.
Wednesdays -- 8:30 a.m. - 12:30 p.m., closed in the afternoon.
Thursday -- 8:30 a.m. - 12:30 p.m.
and 1:30 p.m. - 5:30 p.m.
Friday --
8:30 a.m. - 12:30 p.m. and 1:30 p.m. - 5:30 p.m.
We are closed for lunch 12:30 p.m. - 1:30 p.m. We are closed for all major
holidays.

How do I get there?
Valencia Health Services is located at 1647 Valencia Street,
(between Cesar Chavez and Mission), near St. Luke's Hospital.

VHS is easily reached by MUNI (public transportation). The #26 Valencia
bus (to Duncan and Valencia by St. Luke's Hospital), the #49 Van Ness,
or the #14 Mission (to Mission and Valencia) all stop within one
block of our clinic. We are approximately 3 blocks (10 minute walk)
from the 24th Street BART station.
A parking lot is located next
to the clinic. Please park only in spaces marked for Valencia Health
Services and write your car information next to your child's name
when you sign-in.
How do I make my child an appointment?
For routine check-ups and follow-ups, call and schedule an appointment.
If your child is sick and it is not an emergency, please call us
for advice or an appointment during business hours. We can often
fit your child in to see a clinician the same day. If your child
is sick when the clinic is closed and you think it is urgent, call
415-647-3666 and press 0 to speak with our answering service. A clinician
will call you back.
If you think your child is having a medical emergency,
call 911.

What happens at my first visit?
Please arrive 15 minutes before your appointment time to fill out
new patients forms. These forms include consent for treatment, patient
confidentiality, your personal and insurance information, and history
forms. Bring your insurance card and co-payment (if any) to this
and every visit, as well as your child's immunization record and
any forms you need completed.
Can my child be seen without me?
Parents or legal guardians must accompany all
minor children to their initial appointment and preferably to all
following appointments. We realize that there occasionally may be
times other family members will need to bring your child. In this
situation, please send a signed note granting permission for your
child to be seen that day with the accompanying adult and a phone
number where we may reach you.
