If you think your child is having a medical emergency,
call 911.

- to schedule urgent care
appointments, follow-up visits, and routine check-ups
- to speak with a
staff member, leave a non-urgent message for your provider, or for general information
Q: How do I get forms filled out for my child's school, WIC, etc.?
A:
In order to have forms filled out and/or signed,
please leave them with the front desk when you check in.
Forms will be ready within 2 business days.
