Valencia Health Services offers many general and preventative health services. Below is a complete list of our services. If you have questions about your visit, or are unsure if you should visit VHS, please call us directly at 415-647-3666.

Please arrive 15 minutes before your appointment time to fill out new patients forms. These forms include consent for treatment, patient confidentiality, your personal and insurance information, and history forms. Bring your insurance card and co-payment (if any) to this and every visit, as well as your child's immunization record and any forms you need completed.
