Las situaciones de emergencia

Si su hijo tiene una emergencia médica, llame al 911.

¿Es esta su primera visita al Clínica VHS?

◊   Llegar aqui al menos 15 minutos antes de su cita (tiempo designado) para llenar los formularios para pacientes nuevos.
◊   Traiga su tarjeta del seguro y el pago.
◊   Traiga el comprobante de las vacunas de su hijo(a), cualquier formulario que necesite que llenemos, y el nombre y dirección del último proveedor de salud de su hijo (así podremos pedir información sobre cualquier antecedente médico importante para el cuidado de su hijo(a).
◊   Para la primera visita, el padre o guardián legal deben acompaña su hijo.

Problemas urgentes

◊ Llame VHS en (415) 647-3666 y se darle consejos, o programar una cita para usted.
◊ Cuando la clínica es cerrada, por favor llame el numero de clínica y nuestro servicio de contestadora le ayudará.

Is this your first VHS appointment?

Arrive at least 15 minutes before your appointed time, to complete new patient forms.
Bring your insurance card and co-payment.
Also bring your child's immunization record, any forms you need completed, and the name/address of your child's last healthcare provider (so we can request past medical information important to your child's care).

Urgent Problems

Telephone VHS at (415) 647-3666 and we will give you advice, or schedule an appointment for you.
When we are closed, please call the clinic number and our answering service will help you.

Can my child be seen without me?

For the first visit, the parent or legal guardian MUST accompany their child.
If, for reasons beyond your control, you cannot bring your child to a follow-up or urgent visit appointment, you must send them with:

A responsible ADULT
A signed note giving us permission to see your child with that adult
A telephone number where we can reach you at the time of the appointment
